Forms from Commonwealth of Massachusetts Department of Environmental Protection:

Please note that both a Notice of Intent (NOI) and a Request for Determination of Applicability (RDA) require a Public Hearing. The applicant must submit to the Stockbridge Conservation Commission the appropriate applications, documents, proofs of certified mailings to abutters for NOI applications, and fees to schedule a Public Hearing. Scheduling MUST be done by noon Friday at least 11 days prior to the actual hearing. There is no application fee for the Request for Determination of Applicability, however there is a $60 filing fee to cover the required advertising. The check should be made out to the Town of Stockbridge.

The Stockbridge Conservation Commission STRONGLY recommends that whenever possible you download and read the instructions carefully.
